CHC - Fast Track Walkthrough

CHC2DST  Digital Fast Track form

Once authorised user is signed in:

Click on "New" to complete a new Fast Track application.  The first page is "Patient" details:

The second page is Equality:

The third page is “Documents” that allows the referrer to upload any documents to support the Fast Track application:

The last page is “Recommendation”:

mceclip4.png

Once the form has been submitted the referrer can choose to download a PDF copy of the form:

An email is also sent to the referrer confirming they have submitted the Fast Track form to the CCG:
