CHC Quarterly Report Enhancements

The following changes were made to the CHC reports:

Re-labelled the "Quarterly Export" tab to be "Case Data"
Extended the columns of data on the "Case Data" tab to include these data fields:
a. After the "Ref No" column added in the patient's NHS number
b. After the "created by" column added in the user's Organisation or user's CCG (from User Details)
c. After the addition of the patient's NHS number, added in the "Local Authority" that has been recorded on the Supplementary Information Stage. This will be blank at the moment as we're just finishing off a new product release that will include a field to record the patient's Local Authority that will be recorded on the "Supplementary Information" Stage of the Checklist to MDT Workflow process.
New "Fast Track (Beta)" tab that contains data linked to Fast Track cases. It's labelled as "Beta" as it's only been checked by us at IEG4 and we'd like you to check that you are happy with the content. These are the columns of data:
